The process is simple.
1. Click on GUIDELINES and INSTRUCTIONS (found below) in your preferred format of PDF of Microsoft Word document. Make certain that you meet the qualifications sought by the Champaign County Memorial Foundation found on the page.
2. Click on GRANT APPLICATION (found below)in your preferred format of PDF of Microsoft Word document. Download and print out the application form.
3. Fill out the Grant Application making certain that the signature of the board president/executive director and the treasurer have signed it.
4. Mail the Grant Application to the Champaign County Memorial Foundation at the address found at the end of the form.
